Frequently Asked Questions

Do you take insurance?

I am an out of network provider with private insurance plans. I will provide you with the necessary paperwork to submit claims to your insurance company. However, you may want to contact your insurance company regarding out of network claims submission and if a referral is required for reimbursement. The financial responsibility belongs to the patient at the time of service.

What if I have Medicare or Medicaid?

Unfortunately, if you are Medicare eligible or on Medicaid I will be unable to see you due to government regulations regarding out of network providers.

Do I need a referral?

In the state of Illinois you do not need a referral to be evaluated and treated by a physical therapist. You will need to be under the care of a primary care physician or a chiropractor for compliance under the physical therapy practice act.

How does payment work?

We ask for payment prior to or at the time of your appointment. We accept all major credit cards and cash.

Can I use my HSA or FSA?

We accept payment via your HSA or provide you with the paperwork required to receive compensation from your FSA.

What is your cancellation policy?

Cancellation within 24 hours or no-shows will be charged at full appointment fee. However, we strive to support our clients in every way possible!
