Do you take insurance?

RehabIT Concierge Physical Therapy is in network and a participating provider with with BCBS PPO, Blue Choice PPO and Medicare plans.


RehabIT Concierge Physical Therapy is an out of network provider with all other private insurance plans. I will provide you with the necessary paperwork to submit claims to your insurance company. However, you may want to contact your insurance company regarding out of network claims submission and if a referral is required for reimbursement. The financial responsibility belongs to the patient at the time of service.


Unfortunately I am not credentialed with Medicaid I will be unable to see you due to government regulations regarding out of network providers and Medicaid.

What if I have a Medicare Advantage Plan?

If you have an Medicare Advantage Plan we can discuss your options depending on your specific plan. Each Advantage Plan is unique with different requirements.

Do I need a referral?

In the state of Illinois you do not need a referral to be evaluated and treated by a physical therapist. However, medicare requires a signed order from a Physician (MD or DO).

How does payment work?

We ask for payment prior to or at the time of your appointment. We accept all major credit cards and cash.

Can I use my HSA or FSA?

We accept payment via your HSA or provide you with the paperwork required to receive compensation from your FSA.

What is your cancellation policy?

Cancellation within 24 hours or no-shows will be charged a fee. However, we strive to support our clients in every way possible!

All session are 60 minutes long with the option for 90 minute sessions
